Welcome to the SUB-ACUTE CARE PROGRAM
at the Aberjona, Winchester and Woburn Nursing Centers.
This program is the Salter family’s response to the need
for high quality care in the most appropriate and
effective setting. We offer individual,
specialized therapies for each resident’s unique and
personal goals.
WHY A SPECIAL SUB-ACUTE CARE PROGRAM?
Most
people have the potential to return home after
hospitalization, if given the opportunity to increase
their level of independence through rehabilitative
therapies. The SUB-ACUTE CARE PROGRAM gives
patients the opportunity to regain their confidence and
self-sufficiency.
Upon a patient’s
arrival, a team of rehabilitative specialists will be
assigned. A customized program, based on the
patient’s needs and goals can include:

Referrals to this program are made by the Medical Team
based on your need for specialized services available at
the SUB-ACUTE CARE PROGRAM.
PHYSICIAN'S COVERAGE
To
continue the Treatment Plan initiated by your primary
care physician, he/she will direct that care at the
SUB-ACUTE CARE PROGRAM. The physician's management of
your care is a crucial part of our services.
ABOUT THE SALTER HEALTHCARE CENTERS
The
Nursing Centers are skilled nursing facility owned and
operated by the Salter Family, since 1957. The family is
well known for their leadership in the HEALTHCARE
community.
The staff,
including nurses, therapists and other caregivers are
experienced certified professionals. Patients are assured that their care
will be thoughtful, caring and technically sound.

achieve your goals.
INSURANCES ACCEPTED BY THE SUB-ACUTE CARE PROGRAM
Our
program is covered by most major insurers including,
Medicare, Blue Cross, HMO's, Secure Horizons, Fallon,
U.S. Healthcare, Harvard-Pilgrim and other private
insurer's. Please feel free to discuss the details of
your insurance coverage with the Hospital's Discharge
Planning Department.
HOW TO MAKE A REFERRAL
The
hospital's Discharge Planning Department services as a
liaison between itself and our Nursing Centers,
ensuring a fast and streamlined referral process.
Referrals are accepted twenty-four hours a day., seven
days a week. In fact, an admission can occur as quickly
as one hour from the time of referral.
If you have
a patient at the hospital for whom our SUB-ACUTE CARE
PROGRAM would be appropriate, simply call the Discharge
Planning Department, at the hospital. You may call the
Salter Central Admission Line directly at (781)
729-2200, to discuss a patient, or, obtain more
information.
