Aberjona Rehabilitation and Nursing Center recently announced the start of their Transitions Program, a program designed for patients who are waiting to be discharged from the Center but are not fully ready to live independently again. The program helps patients adjust to home life, but in the safety and supervision of the Transition Suite. Throughout their stay in the Transition Suite, the Center will still be able to monitor the patient’s progress during the final days of recovery.
Patients who do not feel ready to return to independent living have the option to develop their own customized plan of care with Aberjona’s interdisciplinary clinical team.
The Transitions Suite has spared no amenities including a private room and bath, a kitchenette and a dining area. All of these features are designed to assist in simulating the patient’s home environment while he or she remains under the care and supervision of Aberjona’s experienced interdisciplinary clinical team.
