WINCHESTER, MA – Salter HealthCare is pleased to announce that their Woburn Rehabilitation and Nursing Center recently partnered with Honoring Choices® Massachusetts, a nonprofit organization that provides information and programs to patients and their families to help them create personalized health care plans.

The goal of Honoring Choices® Massachusetts is to build a strong network to ensure that adults in Massachusetts have access to early and ongoing health care planning and receive lifelong quality care. The organization provides comprehensive information on health care planning, Massachusetts planning documents and a step-by-step planning process to help adults choose their own health care plan. Partnering with Honoring Choices® will bring new resources to the Center for both residents and staff that will add to the person-centered care approach that is a trademark of all Salter Health Care facilities.

“We’ve dedicated an experienced staff member to champion health care planning and place it at the forefront of patient and family centered care,” Woburn Rehabilitation and Nursing Center Executive Director Cheryl Evangelista said.

Longtime member of Salter HealthCare’s nursing team Pamela Gelardi, RN has become a Certified Honoring Choices® Facilitator. For over ten years, she has served as a full-time nursing supervisor and care plan coordinator in both long and short-term resident care. By working as the Honoring Choices® Facilitator, Gelardi will be assisting residents and families in choosing a health care proxy, exploring options and making end-of-life decisions regarding treatment that may include referrals for palliative care or hospice care.

“Patients and families do not always know that the tools used to communicate their choices can impact so much of their care,” Gelardi said. “We help a person review their individual plan to see what’s right for that person at the current stage of their life and help them look towards the future. We are excited to be working with Honoring Choices® Massachusetts to ensure every person has a voice.”

The Woburn Rehab and Nursing Center’s social services department, nurses and medical team address health care planning and advance directives from admission through discharge. The addition of the Honoring Choices® Massachusetts facilitator role to the facility will only improve their team approach to quality care.

She will provide information, help to document choices, facilitate referrals and help ensure that the patient’s health care decisions are honored.
